Where to Start.....

1. Am I Eligible? 

Holbrook Meals on Wheels Service Inc. (HMOW) provides "More Than Just A Meal!"

We aim to enhance peoples wellness through providing a nutritious and tasty meals service, ensuring adequate nutritional intake, assisting people who use our service to maintain their independence to remain living comfortably and safely in their own home.  We provide a meal delivery for anyone looking for a healthy, delicious, cost-effective meal service as well as providing other in home aged care support services, to help you remain living at home for as long as you choose to do, right across the Greater Hume Council area.


We also provide Other Food services, Social Support assistance, Transport, Domestic Assistance and Personal Care services.

Our services are funded by the Commonwealth Department of Health through the Community Home Support Programme (CHSP) to provide  high quality food and in home aged care support services  to  frail, aged, and older people.  


Our services are subsidised under the following programmes:

  • CHSP 
    • Aged 65 years or over
    • Aged 50 years or over and identify as Aboriginal or Torres Strait Islander
  • Home Care Packages (HCP) (Meals, Meal Preparation and Delivery, social Support & Transport)


2. Register with My Aged Care      

You are eligible to receive government funded service with HMOW if you are:
  • 65 years or older
  • 50 years or over for Aboriginal and Torres Strait Islander people
  • You are still living at home, but need some support to continue to do so.
My Aged Care is the starting point to access Australian Government funded aged care services.
My Aged Care provides:
  • information on the different types of aged care services available
  • an assessment of needs to identify eligibility and the right type of care
  • referrals and support to find service providers that can meet your needs
  • information on what you might need to pay towards the cost of your care

The first step to accessing government-funded aged care is to arrange an assessment through My Aged Care by calling 1800 200 422 or registering online.

A trained Assessor will conduct a face to face assessment with you in your home, to talk about your needs and circumstances. The assessor will tell you what services you are eligible for, or explain to you why you are not eligible. 


If you are approved to receive services through My Aged Care, there are two ways that you can connect with HMOW to organise your services: 

  1. Your assessor can make a referral for you by sending a referral direct to HMOW.
  2. You can refer yourself to HMOW by asking your assessor to give you your referral code(s).
  • Our Home Support Coordinator will then contact you to register you as our client
  • Each service that you are eligible for will have a separate referral code.
  • You can then provide these codes to HMOW.
  • Our team will then be able to register you as a client and start organising your services.  
The process typically takes up to 48 hours, however, we can commence our service to you in the meantime if needed.
 

People With Home Care Packages

Holbrook Meals on Wheels Service is funded under the CHSP programme. 
If you are allocated and accept a Home Care package you are no longer eligible for CHSP funded services, BUT  
YOU CAN STILL CHOOSE TO RECEIVE OUR SERVICES.

We can work with your Home Care Package Provider to offer meals, meal preparation in your home, social support and transport services under
Level 1, 2, 3 and 4 Packages.

How Packages work

  • After assessment and when you have been allocated a package, you will receive notification from My Aged Care.  
  • It is then up to you to choose a Home Care Package Provider (HCCP).
  • When you have chosen a HCCP you will have a consultation session with your HCCP to decide what will be included in your package. 
  • Which services are included in your package is YOUR  choice. Providers are there to assist YOU.
  • You may also be choose to self-manage your funds.
     

I have a Home Care package and I need meals delivered, meal preparation, social support or transport services

  • You will need to state in your Home Care plan that you wish to have meal preparation and meal delivery paid for out of your plan so that we can deliver meals to you, then you just need to contact us.
  • Provision of Social Support and or Transport services, by us, also needs to be approved in your plan.  Please speak to your HCPP to ensure all services you require are in your plan.

Agreement

  • Once your plan has been established we will meet with you to discuss a Service Agreement and options for you, discuss the fees payable and the meal delivery can then begin.
     

What would you have to pay?

  • Level 1, 2, 3 and 4 Packages cover the cost of meal preparation and delivery.
  • The cost price of the meal ingredients is paid by you. Home Care packages are unable to pay for the raw food component of a meal.
